Community and Social Service Specialists - Desk reviews
Work Environment: In office, mostly cubicles.
CONTRACTOR RESPONSIBILITIES:
• *** This position does NOT include any travel. Contractors complete "medical case file reviews" from the office. • What they'll do is review a 17 page assessment for a client already on LTC Medicaid who is residing in the community. • The contractor will then use the information contained in the assessment to determine whether the client still meets the medical criteria for our program Each case will require a detailed write up of the client's history, current circumstances as well as justification as to how/why they still meet medical criteria for the program. • Training will be through job-shadowing and connection with seasoned full time employees. We also have video and online trainings.
• Draws conclusions from reading/reviewing medical records, and then writes corresponding case- notes which highlight the key-points. • Utilizes established guidelines and internal protocol to determine if client meets medical eligibility criteria based on review of medical records and client psychosocial history. • Applies knowledge of aging process, activities for daily living and medical conditions to written case-notes. • Justifies medical eligibility recommendation through written narratives which summarize client’s current situation, limitations and needs. • Presents case-notes and medical eligibility recommendations to Registered Nurse for approval, and adjusts to professional feedback provided by Registered Nurse and/or Supervisor
EDUCATION REQUIREMENTS The candidates must meet the minimum qualification of: • a bachelor’s degree or higher from an accredited college or university with a major in a related field, OR • professional experience totaling four (4) or more years in a related field OR • a combination of education and professional experience totaling four (4) or more years in a related field.
Required/Desired Skills: • Ability to speak and understand English Required • Experience in the social science, human services or a related field, such as case manager or social worker Required 1 Years • Extensive reading and writing competencies Required 2 Years • Knowledge of confidentiality requirements Required • Knowledge of basic medical terminology and electronic records Required 1 Years • Experience using Microsoft Word Required 6 Months • Knowledge of the aging process and effects of chronic illness Required 1 Years • Ability to demonstrate a professional and courteous demeanor to establish and maintain cooperative working relationships with the public and staff Required • Knowledge of medical and social terminology Required 6 Months • Critical thinking Required • Excellent typing skills at least 35 wpm Required |
