• Performs assessments, care and/or treatments in the Family Planning and Sexual Health, Maternity, Immunization, Refugee, and Human Immunodeficiency Virus (HIV) Clinics. • Participates as a team member to obtain and record patients' health and social histories and assess patient needs. Collect patient data and documentation as needed. • Assist Providers as needed for the total care of all patients, assist with discharge plans for clinic patients. • Performs triage assessment for walk-in clinic patients without appointments. Assist with telephone triage. • Requires independent judgment, initiative, applying general knowledge of nursing practices, and instructions to accomplish various job functions. • Provide patient and family education in the Family Planning and Sexual Health, Maternity, Immunization, Refugee, and Human Immunodeficiency Virus (HIV) Clinics. • Receive lab calls and record correct results in the patient medical record. • Review lab consultation reports • Make appropriate disposition as instructed by the provider • Complete medical chart audits • Administer immunizations/vaccines per immunization protocol schedule and perform duties of FL Shots to review, enter and update immunization records. • Schedule follow-up appointments for clients. • Participates in team meetings for discussion and management of patient care. • Participates in planned in-service education sessions and Quality Assurance program and all assigned staff meetings. • Operate equipment used in the clinic, such as electrocardiogram, audiometer, application of oxygen when ordered, nebulizer, and pulse oximeter. • Perform other related nursing duties as assigned. • Work is performed under the supervision of the Senior Community Health Nursing Supervisor or Nursing Consultant. • Perform emergency operations duties during state of emergencies. • Experience in Infectious Disease, Maternity, Sexual Health, Family Planning.
