Case Manager

The Contractor must possess, at minimum, the following qualifications: 
5.1.1. One-year experience working directly with spinal cord and/or brain injured population;
5.1.2. One-year working in the healthcare field providing resource facilitation services.

PURPOSE:
• This statement of Work is for Vocational Rehabilitation Counselor/Adult and Pediatric Case Management services to individuals with brain and /or spinal cord injuries for the Client Division of Emergency Preparedness and Community Support (DEPCS), Bureau of Emergency Medical Oversight (BEMO), Brain and Spinal Cord Injury Program (BSCIP).

CONTRACTOR RESPONSIBILITIES:
6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified:
6.1.1. Perform case closures in accordance with the procedures identified in chapter 10, BSCIP Policy
and Procedures Manual, revised February 2015 as directed by the Department.
6.1.2. Develop a Community Reintegration Plan (CRP) for each client in accordance with the procedures
identified in Chapter 10, BSCIP Policy and Procedures Manual, revised February 2015 as directed
by the Departments.
6.1.3. Authorize and monitor services provided to clients with procedures identified in Chapter 9, BSCIP
Policy and Procedures Manual, revised February 2015 as directed by the Department.
6.1.4. Maintain documentation of interaction with the consumer/family and service providers as
required in Chapter 7, BSCIP Policy and Procedures Manual, Revised February 2015 as directed
by the Department.
6.1.5. Make initial contact with client (consumer)/family within 10 working days of case assignment as
required in Chapter 4, BSCIP Policy and Procedures Manual, Revised February 2015 as directed
by the Department.
6.1.6. Determine program eligibility in accordance with the procedures identified in Chapter 4, BSCIP
Policy and Procedures Manual, Revised February 2015 as directed by the Department.
6.1.7. Conduct program admission for eligible clients in accordance with the procedures identified in
Chapter 5, BSCIP Policy and Procedures Manual, Revised February 2015 as directed by the

Department.
6.1.8. Adhere to the information security and legal procedures identified in Chapter 1, BSCIP Policy and
Procedures Manual, Revised February 2015 as directed by the Department.
6.1.9. Comply with all Department policies and procedures.
6.1.10. Travel when necessary to perform the tasks as directed by the Department.
6.1.11. Submit a time sheet weekly indicating the number of hours worked performing assigned tasks. A
completed BSCIP Weekly Field Accountability Spreadsheet must be submitted to the Contract
Manager. The Field Accountability Spreadsheet will describe the tasks performed.
6.1.12. The Contractor, its employees, subcontractors, and agents must comply at all times with all
Department data security procedures and policies in the performance of this scope of work as
specified in the Data Security and Confidentially document attached to the purchase order.
